Job summary

RiseWell Community Services in Patchogue, NY is seeking a licensed clinical social worker to provide services to children and youth under 21, including individual, group, and family therapy, and to support families in community-based, non-institutional settings. The role emphasizes evidence-based practices, strengthening relationships, teaching coping skills, and coordinating with other providers.

A valid NY LMSW license and a clean driving record are required, with fingerprinting and background

Qualifications

• Licensed Master Social Worker (LMSW) in New York.
• Experience working with individuals in recovery from mental illness.
• Valid and clean NYS Driver's License; fingerprinting and background check.
• Approval from NYS Office of Mental Health.

Responsibilities

• Provide services to children and youth under 21 years old.
• Offer community-based support and services to children in non-institutional settings.
• Deliver individual, group, or family therapy and coordination with other providers.

Job description

Our Children and Family Treatment Support Services, which includes individual, group, or family therapy, family peer support, and rehabilitation services, strengthen children/youth and their families to help them make informed decisions about their care through evidence-based practices. Primary goals include rebuilding relationships, enhancing communication, learning coping skills, incorporating therapy into everyday practice, assessments for mental health and/or substance use needs, and more. In this role, you will provide Services to Children and youth under 21 years old. Offer community-based support and services to children in non-institutionalized settings that enable them to remain at home and in the community. Offer participants independent choices among an array of service options and providers. OLP services will be provided (individual / family therapy).

Qualifications

Licensed Master Social Worker. Experience working with individuals in recovery from Mental Illness. Valid and clean NYS Drivers License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
