Job Summary

A member of the patient care team within the Department of Hospice and directly accountable to the Hospice Director or Patient Care Manager. Indirect supervision of the Program Coordinator. The position requires assisting professional nursing staff by performing nurse aide functions such as bathing, shampooing, ambulation, transfers, tidying sick areas, and other duties as per the care plan under the supervision of a registered nurse or therapist. Demonstrates knowledge and skills to provide age-appropriate care, assesses patient data, and documents patient care accurately. Notifies RN of changes and participates in service and competency programs. Performs other duties as assigned.

Performance Expectations

• Demonstrates ability to bathe patients safely, maintaining privacy and preventing falls.
• Transfers patients safely using proper body mechanics.
• Documents in the EMR according to policies and procedures.

Qualifications

Education/Skills: High School Diploma/GED required. Completion of an approved Nurse Aide training course and a 75-hour certification program as a Home Health Aide or Hospice Aide. Prior Nurse Aide certification preferred.

Work Experience: Six months of nurse aide experience preferred.

Certification/Licensure: Valid Driver's License with proof of insurance. Basic Life Support certification within 30 days of employment required.
