Job description

Job Summary:

A member of the patient care team within the Department of Hospice, directly accountable to the Hospice Director or Patient Care Manager. The position involves assisting professional nursing staff by performing specific nurse aide functions such as bathing, shampooing, ambulation, transfers, maintaining a tidy sick area, and other duties as identified in the care plan, under the supervision of a registered nurse or therapist. The CNA demonstrates the knowledge and skills necessary to provide age-appropriate care, understands principles of growth and development, and can assess patient data to identify individual needs. The CNA also documents patient care accurately and timely, notifies RNs of any changes, participates in service and competency programs, and performs other duties as assigned.

Performance Expectations:

• Ability to bathe patients safely in shower or tub, using appropriate water temperature, preventing falls, and maintaining privacy.
• Ability to transfer patients safely, utilizing proper body mechanics.
• Ability to document care in the EMR according to policies and procedures.

Qualifications:

Education/Skills:

High School Diploma/GED required. Completion of an approved Nurse Aide training course is required. During orientation, successful completion of a 75-hour certification program as a Home Health Aide is necessary. If no prior certificate, must complete a 75-hour certification as a Hospice aide. Completion of a Nurse Aide training from an approved school is preferred.

Work Experience:

Six months of nurse aide experience preferred.

Certification/Licensure - Due Upon Hire:

• Certified Nurse Aide Certificate (preferred)
• Valid Driver's License with proof of automobile insurance

Additional Certification/Licensure:

• Basic Life Support (BLS) within 30 days of employment required
