COMHAR, Inc. in Walnutport, PA, is seeking a Certified Peer Specialist to join our LTSR Behavioral Health program. The position supports individuals in recovery, conducts group activities, and collaborates with the treatment team to advance recovery goals.

The role focuses on education, daily living skills, community integration, and wellness. Shift schedule is Monday, Wednesday, and Friday, 9:00am–4:00pm, totaling 20 hours weekly, with a pay rate of $17.36 per hour.

Qualifications

• HS/GED is required.
• Certified Peer Specialist Certification is required.

Responsibilities

• Assist in developing and implementing educational and support groups and program activities.
• Provide training to individuals on recovery skills and coping strategies.
• Assist with activities of daily living and self-care skills.
• Support social skills, vocational training, community inclusion, and wellness.
• Participate in treatment team meetings and help individuals work towards recovery goals.
• Co-facilitate peer-led meetings and participate in peer-driven initiatives.
• Accompany individuals to medical appointments and other activities in the community.
• Assist with meal preparation and adhere to dietary restrictions per plan.
• Complete resident rounds and ensure safety and location accountability.
• Provide shift reports to ensure continuity of care and accurate records.
• Assist with facility upkeep and light cleaning as needed.
• Report crises and incidents to supervisors promptly.
• Document progress and status in clinical records and comply with regulations.
• Participate in Quality Improvement activities and training sessions.
• Adhere to COMHAR policies and procedures.

Our mission is to provide health and human services that empower individuals, families and communities to live healthier, self-determined lives. We are seeking a Certified Peer Specialist to join our LTSR Behavioral Health program in Walnutport, PA.

Location: Walnutport, Northampton County

Reports to: Director LTSR

Responsibilities

• Assists in the development and implementation of educational and support groups, program activities, and other programming aspects as needed.
• Provides training and assistance to individuals in developing and practicing the skills identified and outlined in their recovery plans. Teaches the proactive use of positive coping strategies.
• Provides training and assistance in activities of daily living and related self-care skills. Areas include bathing, toileting, personal hygiene, grooming, dressing, care of clothing, and shaving.
• Provides training and assistance in social skill building, vocational training and employment, community inclusion, and health and wellness. Areas of focus include leisure planning, transportation, meal preparation, housekeeping, shopping, scheduling appointments, fiscal management, etc.
• Participates in treatment team meetings as assigned. Assists individuals in working towards the completion of their recovery goals as written on their treatment plan.
• Co‑facilitates meetings to nurture a peer‑supported culture via community meetings and other peer‑run initiatives.
• Accompanies individuals in recovery into the community to medical appointments, family visits, and other appointments.
• Assists with meal preparation and clean up as needed. Follows individual dietary restrictions/special diets during mealtimes as prescribed.
• Completes resident “rounds” as assigned to ensure that all individuals in recovery are safe and in their expected locations.
• Provides a comprehensive shift report at shift change to ensure that all meaningful information is passed on for proper continuity of care of individuals in recovery.
• Assists with the general upkeep of the facility and participates in light cleaning and household chores.
• Timely reporting of all crises and unusual incidents to Shift Supervisor, Nurse Manager and/or LTSR Director (Immediately if indicated; otherwise within 24 hours).
• Completes detailed documentation for individuals in their clinical record and in other areas as directed. Documentation should be timely, legible, and provide an accurate depiction of progress, challenges, medical status, etc. Satisfies compliance with regulations regarding record keeping and reporting.
• Participates in Quality Improvement Program as assigned.
• Attends all meetings and training sessions as assigned.
• Maintains compliance with all COMHAR policies and procedures with no unauthorized exception.
