Job summary

Responsibilities

  • Assists in the development and implementation of educational and support groups, program activities, and other programming aspects as needed.
  • Provides training and assistance to individuals in developing and practicing the skills identified and outlined in their recovery plans.
  • Teaches the proactive use of positive coping strategies.
  • Provides training and assistance in activities of daily living and related self-care skills such as bathing, toileting, personal hygiene, grooming, dressing, and shaving.
  • Supports social skill building, vocational training and employment, community inclusion, and health and wellness activities.
  • Participates in treatment team meetings and assists individuals in working toward recovery goals per their treatment plan.
  • Co-facilitates meetings to nurture a peer supported culture via community meetings and other peer-led initiatives.
  • Accompanies individuals in recovery to medical appointments, family visits, and other engagements.
  • Assists with meal preparation and follows dietary restrictions.
  • Completes resident rounds to ensure safety and location tracking.
  • Provides shift reports to ensure continuity of care and accurate progress notes.
  • Assists with facility upkeep and light cleaning duties.
  • Reports crises and incidents promptly to supervisors and directors (within 24 hours if not immediate).
  • Completes detailed documentation in clinical records and other areas as directed, ensuring timeliness and accuracy.
  • Participates in Quality Improvement activities and attends meetings and trainings.
  • Maintains compliance with COMHAR policies and procedures.

Shift Schedule: Monday, Wednesday and Friday 9:00am-4pm (20 Hours)

Pay Rate: $17.36/hr

Our mission is to provide health and human services that empower individuals, families and communities to live healthier, self-determined lives. We are seeking a Certified Peer Specialist to join our LTSR Behavioral Health program in Walnutport, PA

Location: Walnutport, Northampton County

Reports to: Director LTSR

Responsibilities:

  • Assists in the development and implementation of educational and support groups, program activities, and other programming aspects as needed.
  • Provides training and assistance to individuals in developing and practicing the skills identified and outlined in their recovery plans. Teaches the proactive use of positive coping strategies.
  • Provides training and assistance in activities of daily living and related self-care skills. Areas include bathing, toileting, personal hygiene, grooming, dressing, care of clothing, and shaving.
  • Provides training and assistance in social skill building, vocational training and employment, community inclusion, and health and wellness. Areas of focus include leisure planning, transportation, meal preparation, housekeeping, shopping, scheduling appointments, fiscal management, etc.
  • Participates in treatment team meetings as assigned. Assists individuals in working towards the completion of their recovery goals as written on their treatment plan.
  • Co-facilitates meetings to nurture a peer supported culture via community meetings and other peer run initiatives.
  • Accompanies individuals in recovery into the community to medical appointments, family visits, and other appointments.
  • Assists with meal preparation and clean up as needed. Follows individual dietary restrictions/special diets during mealtimes as prescribed.
  • Completes resident “rounds” as assigned to ensure that all individuals in recovery are safe and in their expected locations.
  • Provides a comprehensive shift report at shift change to ensure that all meaningful information is passed on for proper continuity of care of individuals in recovery.
  • Assists with the general upkeep of the facility and participates in light cleaning and household chores.
  • Timely reporting of all crises and unusual incidents to Shift Supervisor, Nurse Manager and/or LTSR Director (Immediately if indicated; otherwise within 24 hours).
  • Completes detailed documentation for individuals in their clinical record and in other areas as directed. Documentation should be timely, legible, and provide an accurate depiction of progress, challenges, medical status, etc. Satisfies compliance with regulations regarding record keeping and reporting.
  • Participates in Quality Improvement Program as assigned.
  • Attends all meetings and training sessions as assigned.
  • Maintains compliance with all COMHAR policies and procedures with no unauthorized exception.

Requirements:

  • HS/GED + Certified Peer Specialist Certification
