The Fortune Society, in partnership with CHN, seeks a Certified Peer Navigator (CRPA) for a day shift in Brooklyn/Queens area. The role includes in-court collaboration, initial screenings, and immediate service linkage to clinical teams and resources.

Emphasis on compassionate outreach and rapid needs assessment. The position requires CRPA certification, prior peer experience, and flexibility to work Wednesday–Sunday with rigorous documentation and community coordination.

Competenze

CRPA certification required; continuous peer support experience preferred.
Bilingual in Spanish a plus; valid driver's license required.
High school diploma or equivalent required; willingness to obtain further credentials may be considered.

Mansioni

Screen and engage individuals leaving arraignment courts with medical or mental health needs.
Coordinate with Fortune care manager and MMU clinical team to determine immediate level of care.
Obtain consents, verify insurance, and collect baseline demographics for the care team.
Arrange connections to housing, detox, healthcare, or other needed services; provide referrals as needed.
Complete documentation in the case management database in a timely manner; ensure follow-ups.
