Continuum Rehab Group seeks a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

• Licensed OTA credential required.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under supervision.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising OT to adjust treatments.
• Provides counseling to patients and families related to physical disorders.
• Contributes to discharge summaries per regulatory and clinical requirements.
