Job summary

Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. This role focuses on delivering evidence-based, skilled therapy under supervision of an Occupational Therapist in a compassionate, clinically driven setting.

The position offers mentorship, professional growth, and a pathway for specialty certification paid for by CRG, along with a comprehensive benefits package and licensure

Qualifications

• Licensed Occupational Therapy Assistant licensure.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient’s progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
