Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We offer a comprehensive compensation package including benefits, licensure reimbursement, and a mentorship program to support clinical growth.

Licensed OTAs will deliver skilled therapy under OT supervision, participate in evidence-based practice, and contribute to patient recovery and quality of life.

Qualifications

• Licensed Occupational Therapy Assistant credential required.
• Open to therapists at all experience levels.
• Experience with evidence-based practice preferred.

Responsabilités

• Utilize evidence-based interventions to guide treatments.
• Provide skilled therapy and rehabilitative services under supervision of an Occupational Therapist.
• Assess patient progress and adjust treatment plans accordingly.
• Communicate with supervising OT and care teams to optimize discharge planning.
• Provide education and counseling to patients and families.

Experience/Qualifications:
Licensed Occupational Therapy Assistant This position is open to therapists at all experience levels.
Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
