Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. This role focuses on delivering evidence-based rehabilitative services under the supervision of an Occupational Therapist.

Responsibilities

• Uses evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under OT supervision.
• Delivers skilled therapy services to patients needing rehab.
• Assesses progress and communicates with supervising OT to adjust treatments.
• Provides consultation to patients, families and caregivers on physical disorders.
• Assists with discharge planning and documents progress per guidelines.

Experience/Qualifications:

Licensed Occupational Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient’s progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
