Job summary

Continuum Rehab Group seeks a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Las Vegas, NM. You will provide evidence-based rehabilitation under supervision and contribute to individualized care plans, documentation, and patient progress.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders under supervision of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patients' progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients and families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
