Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Massena, NY. Full-time, part-time or PRN positions are available.

The OTA will plan, organize, develop and direct occupational therapy services to ensure high-quality resident care, under supervision of the licensed therapist. Must have an Associate's degree in Occupational Therapy and active OTA licensure.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Screen residents to determine best intervention.
• Perform treatment plans per established plan of care.
• Interpret and communicate evaluation findings to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with team members.
• Document and bill occupational therapy services as per plan of care.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily.

Education
