Flagship Rehabilitation in Middletown, PA is seeking a Certified Occupational Therapy Assistant - Per Diem to join our SNF and long-term care setting. Must be a COTA to apply, with an hourly compensation of $40.00-$45.00.

You will work under OT supervision to implement care plans and ensure regulatory compliance. Join a team with over 30 years of excellence in care, mentorship, and ongoing clinical education.

Qualifications

• Associate’s degree from an AOTA-accredited Occupational Therapy Assistant program.
• NBCOT COTA certification required by state (if applicable).
• Current license as a Certified Occupational Therapy Assistant in state of practice.
• Strong communication and organizational skills.

Responsibilities

• Provide occupational therapy treatments under supervision of an OT.
• Assist in implementing personalized care plans focused on functional goals.
• Collaborate with team to ensure quality outcomes.
