Job summary

HealthMark Rehabilitation is seeking a PRN Certified Occupational Therapy Assistant (COTA) for Park Manor of South Belt in Houston, Texas. The role delivers skilled therapy to residents under physician orders and participates in plan-of-care development.

Candidates must hold Texas COTA licensure, demonstrate strong communication, organizational and analytical skills, and meet physical requirements. Prior geriatric rehab experience is preferred, with background check included.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develops an overall plan of care including treatment, goals and frequency to support discharge planning.
• Performs skilled treatments according to plan of care.
• Documents observations and patient response to interventions and progress.
• Completes all documentation timely and in accordance with state and federal regulations.
• Completes billing for the date of service in compliance with regulations.
• Maintains rehab gym equipment and treatment area with proper infection control.

We’re currently seeking a PRNCOTAto treat residents and short-term patients at our Park Manor of South Beltfacility in Houston, Texas.

Responsibilities:

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Registered therapists, develop an overall therapy plan of care to include: treatment and medical diagnosis warranting skilled intervention, planned duration and frequency to be provided, short and long term goals functional and measurable that support the discharge plan; per standards of practice, and established guidelines.
• Therapists of all disciplines perform skilled treatments according to established plan of care.
• Documents observations, patient’s response to skilled interventions/treatment programs, degree of progress and notable changes in patient’s condition within specific timeframes.
• Completes all documentation required timely and in accordance with state and federal regulations and policies/guidelines.
• Completes all billing required on the date of the provided skilled services, and in accordance with all state and federal regulations, policies/guidelines.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

Requirements:

• Must hold Texas license/certification for appropriate discipline: COTA.
