Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to provide patient-centered therapy under supervision of an Occupational Therapist. The COTA will help carry out prescribed treatments, document progress, and communicate with the OT to adjust plans as needed.

Candidate should be licensed as a COTA, with experience in long-term or geriatric care preferred, and possess strong documentation skills to ensure timely weekly progress notes.

Qualifications

• Must hold a valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed in documentation writing.
• Document patient progress weekly in a timely fashion.

Responsibilities

• Carry out best practice treatments under supervision of the OT.
• Provide treatment aligned with goals developed by the OT.
• Provide feedback to supervising OT via documentation or other required methods.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Report to work on time and adhere to scheduled hours.
• Perform duties assigned by the Director of Rehabilitation.

Certified Occupational Therapy Assistant (COTA)

The Certified Occupational Therapist Assistant is responsible for carrying out best practice treatments under the supervision of the Occupational Therapist. The Occupational Therapist Assistant will provide treatment by adhering to specific goals developed by the OT. As well, the COTA must provide feedback to the supervision occupational therapist via documentation or in any other method required by facility policy.

Qualifications/Requirements:

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Document patient's progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
