Job summary

Southwestern Nursing and Rehab, PA is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our in-house therapy team. This PRN role supports our Occupational Therapists in delivering high-quality rehabilitative care that enhances residents’ independence, functional abilities, and overall quality of life.

The COTA will assist with treatment plans, provide therapeutic exercises, document progress, and collaborate with a multidisciplinary team in a skilled nursing facility

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) license/certification in good standing.
• Experience in a skilled nursing facility (SNF) or rehabilitation setting preferred.
• Strong communication, interpersonal, and organizational skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.

Responsibilities

• Assist the Occupational Therapist in implementing individualized treatment plans.
• Provide therapeutic exercises and activities to improve residents’ functional and daily living skills.
• Accurately document treatments, progress notes, and resident responses in compliance with facility, state, and federal guidelines.
• Participate in interdisciplinary care plan meetings and contribute to resident goal setting.
• Educate residents, families, and caregivers on strategies to support independence and safety.
• Maintain therapy equipment to ensure cleanliness, safety, and proper functioning.
• Adhere to all regulatory, ethical, and facility standards of care.

This full-time role supports our Occupational Therapists in delivering high-quality rehabilitative care that enhances residents’ independence, functional abilities, and overall quality of life.
