Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join its team on a per diem basis. You will support therapists, develop and implement individualized OT plans, and help residents regain independence.

The position emphasizes ADLs, mobility, cognitive skills, and collaboration with the care team to promote recovery and well-being. A Wisconsin COTA license is required, with LTC experience preferred.

Qualifications

• Current certification as a COTA in Wisconsin.
• High school diploma or equivalent; associate's degree or higher in Occupational Therapy Assistant preferred.
• Experience in a long-term care or rehabilitation setting preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Assist Occupational Therapists in the development and implementation of occupational therapy treatment plans.
• Conduct therapy sessions with residents, focusing on activities of daily living (ADLs), mobility, and cognitive skills.
• Monitor and record patient progress, providing recommendations for adaptations or modifications as needed.
