Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities.

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient progress and communicates with supervising OT to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families and caregivers.
• Provides input to supervisor for discharge summary of services provided.

Experience/Qualifications

Licensed Occupational Therapy Assistant

This position is open to therapists at all experience levels.

Some Duties For This Position Include

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
