Job summary

Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing and Long Term Care facilities. You will deliver skilled OT support under supervision and contribute to patient recovery and quality of life.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of an OT.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress; communicates with supervising OT to adjust treatments.
• Provides counseling to patients and families regarding physical disorders.
• Contributes to discharge planning and summary of services.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient's progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
