Job summary

Heritage Place Care & Rehabilitation in Scenic Heights, TN, seeks a compassionate Certified Occupational Therapy Assistant to join our in-house rehab team, delivering high-quality care to residents in a skilled nursing setting.

Under supervision of the Licensed OT, you’ll implement treatment plans, document progress, and collaborate with the rehab team to support residents’ functional goals and quality of life.

Qualifications

• Active State Licensure or certification required.
• Experience in skilled nursing or rehab setting preferred.
• Ability to document and communicate progress clearly.

Responsibilities

• Treat patients according to the plan of care under the supervision of the Licensed OT.
• Communicate with the OT any pertinent information concerning patient progress and plans.
• Record treatment notes and discharge summaries; complete daily billing of services.
• Attend Plan of Care meetings when requested by the Director of Rehab.
• Adhere to state practice acts and company policies and code of conduct.

The primary purpose of this position is to plan, organize, develop, and assist in facilitating our Occupational Therapy Services in accordance with applicable federal, state, and local standards, guidelines, regulations to assure that the highest degree of quality care can be always maintained.

COTA- Jobs and Responsibilities:

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Occupational Therapist.
• Communicate with the Occupational Therapist any pertinent information concerning patient progress, concerns, and plans.
• Be proficient in therapeutic needs to treat patients based on their presentation and instructions given by the Occupational Therapist
• Participate in patient Plan of care meetings if asked by the Director of Rehab.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedure
• Adherence to State Required practice acts, code of conduct, ethics during professional practice as an Occupational Therapy Assistant
• Adherence to Company requires policy, procedures and code of conduct
• Active State Licensure or certification needed
