Job summary

Fallsview Rehabilitation & Nursing is seeking a Full-time Occupational Therapist Assistant (OTA) to join our caregiving team in New Jersey. PRN availability is also noted.

The role focuses on supporting the Occupational Therapist with assessments, ADL training, and continuous patient progress in a warm, community-oriented setting. • Must have New Jersey state OT license.

Responsibilities

• Assist the Occupational Therapist in assessing residents' functional abilities and collecting objective data.
• Implement intervention plans, including ADL training, under the guidance of the Occupational Therapist.
• Monitor residents' performance and report changes to the supervising Physical Therapist.
• Maintain accurate and timely documentation in EMRs (Net Health/PCC) and PoS care delivery.
• Educate residents, family members and care staff on safe mobility techniques and exercises.
• Conduct remote check-ins through remote therapeutic monitoring (RTM) for discharged patients.
• Collaborate with rehab staff, including Physical Therapists and Speech-Language Pathologists.

Responsibilities:

Assist the Occupational Therapist in assessing residents

functional abilities and gathering objective data for collection during Annual, Quarterly, Falls, Significant Change assessments.

Treatment Support: Implement intervention plans, including ADL training, based on the guidance of the Occupational Therapist.

Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.

Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service(PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).

Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.

Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, to provide comprehensive care.

Must have state license in New Jersey
