Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Honey Grove, TX. You will work under supervision to deliver rehabilitative services, track patient progress, and contribute to discharge planning.

Qualifications

• Licensed Occupational Therapy Assistant required.
• Open to therapists at all experience levels.
• Experience in Skilled Nursing/Long Term Care settings preferred.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under the supervision of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient's progress and communicates with supervising OT to adjust treatments accordingly.
• Provides consultation and counseling to patients, families, and caregivers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient's progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
