Job summary

COA Network Inc. is seeking a dedicated Occupational Therapy Assistant to support older adults in outpatient and home-based settings in San Jose, CA.

You will implement individualized treatment plans, assist with evaluations, and help clients gain mobility, balance and independence in daily activities. Collaborate with physicians, therapists and nurses to ensure seamless care, document interventions accurately, and contribute to a high-standard, patient-centered program that values

Qualifications

• Associate's Degree from an OTA program is required.
• Active CA COTA license or eligibility to obtain one.
• Experience providing therapy services to frail or medically complex older adults.
• 1–2 years OTA experience preferred.
• Experience in Outpatient Rehab, Acute Care, Inpatient Rehab or SNF.
• Adult/geriatric patient experience preferred; pediatric experience not considered.

Responsibilities

• Implement individualized occupational therapy treatment plans developed by the Occupational Therapist.
• Assist with participant evaluations, including vital signs, ROM, and strength assessments.
• Provide therapeutic interventions to improve mobility, balance, endurance, cognition, perceptual-motor skills, and ADLs independence.
• Coordinate participant treatment schedules, therapy visits, and related documentation.
• Collaborate with physicians, therapists, nurses, and other interdisciplinary team members to support comprehensive care.
• Document therapy interventions accurately and in compliance with clinical and regulatory standards.
• Provide therapy services in both outpatient center and home-based settings as needed.

• Coordinate participant treatment schedules, therapy visits, and related documentation.
• Collaborate with physicians, therapists, nurses, and other interdisciplinary team members to support comprehensive care.
• Document therapy interventions accurately and in compliance with clinical and regulatory standards.
• Provide therapy services in both outpatient center and home-based settings as needed.

Experience you will need:

• Associate's Degree from an accredited Occupational Therapy Assistant (OTA) program.
• Active California Certified Occupational Therapy Assistant (COTA) License or proof of license eligibility.
• Knowledge and experience providing therapy services to frail or medically complex older adults.
• 1 to 2 years of Occupational Therapy Assistant experience preferred.
• Experience in one or more of the following settings: Outpatient Rehabilitation, Acute Care, Inpatient Rehabilitation, Skilled Nursing Facility (SNF).
• Adult and/or geriatric patient experience strongly preferred.
• Candidates with only pediatric experience will not be considered.
