Job summary

COA Network Inc. in Modesto, CA is seeking a licensed Occupational Therapist to conduct comprehensive clinic evaluations and occasionally provide home health visits to support aging in place.

You will develop individualized treatment plans, participate in IDT meetings, and collaborate with physicians, nurses, and therapists to deliver high-quality care. Active CA OT license required; 1–2 years experience preferred.

Qualifications

• Bachelor's Degree in Occupational Therapy from an accredited program (Master's degree preferred).
• Active California Occupational Therapist License or license eligibility.
• CPR and First Aid certification upon hire preferred, or ability to obtain within 180 days of hire.
• 1–2 years of Occupational Therapy experience preferred.
• Experience in Outpatient, Acute Care, SNF, Inpatient Rehab,/Home Health settings preferred.
• Experience treating adults/geriatrics.
• Strong interdisciplinary collaboration and patient education skills.

Responsibilities

• Conduct comprehensive occupational therapy evaluations in the clinic and occasionally in participants' homes.
• Develop individualized treatment plans based on participant needs, goals, and functional abilities.
• Participate in interdisciplinary team (IDT) meetings to develop and coordinate comprehensive care plans.
• Deliver evidence-based occupational therapy interventions to improve strength, mobility, balance, cognition, and independence with ADLs.
• Provide active and passive therapeutic exercises, muscle re-education, gait training, and functional rehabilitation.
• Supervise and mentor Occupational Therapy Assistants (COTAs), providing guidance on Functional Maintenance Programs (FMPs).
• Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards.
• Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to ensure high-quality participant care.
