Job summary

Personal Touch Home Care of Virginia Inc is hiring a Certified Occupational Therapy Assistant (COTA) for Newport News and Chesapeake, VA, with a sign-on bonus. The COTA will implement standards of care for occupational therapy and ensure adherence to contract terms.

The position is full-time or PRN with a pay range of $38–$43 per hour, serving Southside and Peninsula service areas. Responsibilities include patient evaluation, care planning, teaching, daily living activities training,

Qualifications

• Licensed as an occupational therapist where practicing.
• Graduate from ACOTE-accredited OT education program.
• Good verbal and written communication and organization skills.

Responsibilities

• Evaluate patient status and home environment to plan care.

• Teach or retrain patient in daily activities as needed.
• Fabricate splints and instruct use of adaptive equipment.
• Train on prosthetic/orthotic devices when indicated.
• Maintain clinical records and reports per policy.
• Coordinate with care team for integrated plans.
• Participate in meetings and discharge planning.
• Identify needs for other home health services.
• Instruct family/caregivers on treatment regime.
• Refer to additional services when appropriate.

We are seeking a qualified Certified Occupational Therapy Assistant (COTA)for our Southside service area to include Norfolk, Portsmouth, Chesapeake, Virginia Beach and Suffolk service areas. We are also hiring for the Peninsula service areas.

JOB SUMMARY:

The COTA is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Contractual Services Agreement (when applicable).

RESPONSIBILITIES:

• 1 . Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.) Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
• 2. For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized outside training.
• 3. Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Organization professionals.
• 4. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.
• 5. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
• 6. May train patient in the use of prosthetic and/or orthotic devices.
• 7. Maintains appropriate clinical records, clinical notes, summary reports and reports to the physician any changes in the patient’s condition. Submits these records in accordance with policy.
• 8. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
• 9. Attends and contributes to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the Organization.
• 10. Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
• 11. Instructs patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.
• 12. Participates in discharge planning for patient.

QUALIFICATIONS:

• Licensed or otherwise regulated, if applicable, as an occupational therapist by the State in which practicing.
• Graduate after successful completion of an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA) or successor organizations of ACOTE.
• Demonstrates good verbal and written communication and organization skills.

License/Certification:

Certified Occupational Therapy Assistant License (COTA) (Required)

• BLS CPR Certification
