Job summary

Creekside Village is seeking a full-time COTA to join our rehabilitation team in Hagerstown, Maryland. You will provide comprehensive therapy under the supervision of an Occupational Therapist and help identify patients who can benefit from rehab services.

You will document patient progress weekly, obtain OTR co-signature, and ensure orders and discharges meet MD guidelines. This role requires strong documentation and teamwork within the rehab department.

Qualifications

• Must hold a current COTA license.
• Ability to work under OT supervision and follow state guidelines.
• Strong documentation and communication skills.

Responsibilities

• Provide comprehensive treatment to patients under the supervision of an OTR.
• Assist OT in identifying suitable patients and refer to rehab as needed.
• Document progress weekly and obtain OTR co-signature.
• Ensure MD orders for treatments, recertifications, and discharges are secured.
• Complete documentation per state guidelines and assist with other duties as assigned.

DUTIES AND RESPONSIBILITIES:

• Provide comprehensive treatment to patients under the supervision of an OTR and according to state guidelines.
• Assist OT in identifying appropriate patients and referring them to rehab accordingly.
• Document patient’s progress on weekly progress note in an accurate and timely fashion, and obtain the co-signature of the OTR.
• Ensure MD orders are obtained for treatments, recertification, and discharges.
• Assist with other documentation requirements as indicated and according to state guidelines.
• Attend facility and rehab meetings when indicated by the supervisor.
• Perform all other duties assigned by the department supervisor, lead therapist, or regional manager

QUALIFICATIONS:

• Must have current COTA license
