Job description

The Phoenix Center is seeking a Full-Time COTA- Occupational Therapist Assistant to join our team!

DUTIES AND RESPONSIBILITIES:

  1. Provide comprehensive treatment to patients under the supervision of an OTR and according to state guidelines.
  2. Assist OT in identifying appropriate patients and referring them to rehab accordingly.
  3. Document patient’s progress on weekly progress note in an accurate and timely fashion, and obtain the co-signature of the OTR.
  4. Ensure MD orders are obtained for treatments, recertification, and discharges.
  5. Assist with other documentation requirements as indicated and according to state guidelines.
  6. Attend facility and rehab meetings when indicated by the supervisor.
  7. Perform all other duties assigned by the department supervisor, lead therapist, or regional manager.

QUALIFICATIONS:

  1. Must have current COTA license.
  2. One year of related experience.
