Lake City Healthcare and Rehabilitation in Lake City, FL is seeking a Certified Occupational Therapist Assistant (COTA) to join our rehabilitation team on a full-time basis. The COTA will work under occupational therapists to help residents regain strength, mobility, and independence.

Ideal candidates are licensed in Florida, dedicated to resident-centered care, and comfortable collaborating with a multidisciplinary team to create personalized treatment plans and document progress in accordance

• Active and current Florida Certified Occupational Therapist Assistant (COTA) license required
• Passion for helping residents achieve rehabilitation goals
• Experience in long-term care, skilled nursing, or rehabilitation settings preferred but not required

Responsibilities

• Collaborate with Occupational Therapists and the interdisciplinary team to implement personalized treatment plans designed around each resident’s goals and needs
• Help residents improve their ability to perform daily activities, regain functional skills, and achieve greater independence and quality of life
• Provide skilled assistance during therapy sessions to support recovery from illness, injury, or other health challenges
• Maintain accurate documentation while ensuring resident information is handled with the highest level of confidentiality and in accordance with regulatory standards
• Work closely with nursing, therapy, and interdisciplinary team members to deliver exceptional care and achieve positive resident outcomes

Key Responsibilities

• Individualized Care & Treatment Support: Collaborate with Occupational Therapists and the interdisciplinary team to implement personalized treatment plans designed around each resident’s goals and needs.

• Restoring Independence: Help residents improve their ability to perform daily activities, regain functional skills, and achieve greater independence and quality of life.

• Hands-On Therapeutic Care: Provide skilled assistance during therapy sessions to support recovery from illness, injury, or other health challenges.

• Documentation & Compliance: Maintain accurate documentation while ensuring resident information is handled with the highest level of confidentiality and in accordance with regulatory standards.

• Team Collaboration: Work closely with nursing, therapy, and interdisciplinary team members to deliver exceptional care and achieve positive resident outcomes.
