Transitions At Home is seeking a motivated and reliable Certified Occupational Therapist Assistant (COTA) to help patients regain independence in their homes. We value one-on-one care and a supportive team environment with flexible scheduling.

As a COTA, you will deliver services per physician orders under supervision, with a part-time emphasis (20–29 hours/week, 8:00 am - 4:30 pm, Monday–Friday). This role offers competitive wages and a robust benefits package.

Qualifications

• Active Wisconsin Certified Occupational Therapist Assistant License.
• Two years’ experience preferred.
• CPR Certification.
• Valid Wisconsin Driver's License and Car Insurance.

Responsabilités

• Provide occupational therapy services to patients in their home.
• Perform interventions under the supervision of an Occupational Therapist.
• Support patient independence and safety in the home setting.

What does a Transitions at Home Certified Occupational Therapist Assistant do?

An Certified Occupational Therapist Assistant (COTA) provides occupational therapy services to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Occupational Therapist.

Hours: Part-Time

• 20-29 hours per week 8:00 am - 4:30pm
• Monday through Friday

Qualifications / Job Requirements:
