Job summary

COUNTRY OAKS CARE CENTER is seeking a dedicated Occupational Therapy Assistant to support OT services in a post-acute care setting. The OTA will document services, assist in implementing the OT Plan of Care, and ensure care follows professional standards and state/federal guidelines.

Under supervision of a licensed therapist, you will collaborate with the rehab team, complete daily notes, assist in discharge summaries, and participate in ongoing training to grow clinically.

Qualifications

• Minimum of Associate of Arts in Occupational Therapy Assistant.
• Licensure/Certification: Licensure as required by State guideline.
• Certification as required by State.
• CPR Certification.

Responsibilities

• Provides Therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
• Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with OTR regarding changes in treatment or condition in a timely manner.
• Completes documentation including daily notes and assists the OTR to create progress notes, discharge summaries, or monthly reports as needed.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, referring physicians and other members of the team.
• Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Participates in care planning meetings, rehab meetings and family training in order to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
• Follows organizational and facility policies and procedures to ensure employee and patient safety.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Occupational Therapy Assistant will assist in providing appropriate aspects of quality patient care, under the supervision of licensed therapists. • Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Education Required

• Minimum of Associate of Arts in Occupational Therapy Assistant
• Licensure/Certification: Licensure as required by State guideline
• Certification as required by State
• CPR Certification

Experience

• 1 year Occupational Therapy Assistant experience preferred.
• 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
• Graduates in Occupational Therapy Assistant Program will be considered.
