Job summary

Heritage Manor Care in Crescent City, California is seeking a dedicated Occupational Therapy Assistant to join our post-acute care team. The role focuses on delivering therapy services under supervision, collaborating with an OTR, and contributing to patient progress within state guidelines.

Candidates should have an OTA credential at Associate level, CPR certification, and be willing to maintain licensure.

Qualifications

• 1 year Occupational Therapy Assistant experience preferred.
• CPR certification required.
• Licensure as required by State guidelines.
• Graduates in OTA program will be considered.

Responsibilities

• Provides Therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
• Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with OTR regarding changes in treatment or condition in a timely manner.
• Completes documentation including daily notes and assists the OTR to create progress notes, discharge summaries, or monthly reports as needed.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, referring physicians and other team members.
• Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Participates in care planning meetings, rehab meetings and family training to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
• Follows organizational and facility policies and procedures to ensure employee and patient safety.
• Ensures equipment is in good working order prior to providing care to patients; reports damaged equipment.
• Maintains a clean environment.
• Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

• 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
• Graduates in Occupational Therapy Assistant Program will be considered.
