Job summary

StoneBridge Senior Living is seeking a Certified Nursing Assistant to support residents and medical staff in delivering compassionate care in long-term care settings. You will assist with activities of daily living, monitor residents, and document care under supervision.

Qualifications

  • Certified Nursing Assistant in good standing or graduate of a state-approved nursing assistant training program and passed a state-approved competency exam within 120 days of employment
  • High School Diploma or GED preferred
  • Current CPR/BLS certification preferred
  • At least one year of CNA experience in long-term care preferred
  • Able to express self clearly in written and oral communication in an interdisciplinary care setting

Responsibilities

  • Provides supportive services to residents and other staff as needed and performs duties as assigned
  • Assists residents with activities of daily living according to care plans and policies
  • Assists residents with lifting, turning, moving, positioning, and transporting
  • Coordinates dining room services at mealtimes and assists with feeding
  • Delivers nutritional supplements and documents intake
  • Monitors weight and skin condition and reports changes to nurse
  • Supports toileting program activities and completes flow sheets daily
  • Keeps nurse and supervisor informed of factors affecting performance
  • Reports changes in resident condition to nurse and supervisor

A Certified Nursing Assistant is responsible for supporting medical staff in administering care to residents with various injuries, illnesses or disabilities. The position requires patience, compassion, and a desire to care for the elderly in a gentle and empathetic manner.

Responsibilities and Duties
  • Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
  • Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
  • Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
  • Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
  • Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
  • Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
  • Assists nursing staff in carrying out toileting program activities.
  • Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
  • Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
  • Reports changes in a resident’s condition to the resident’s nurse and supervisor.
Qualifications and Skills
  • Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
  • High School Graduate/GED preferred.
  • Current CPR/BLS certification preferred.
  • At least one year of experience as a certified nursing assistant in long-term care preferred.
  • Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
