Job summary

StoneBridge Senior Living in Jefferson City, MO, is seeking a Certified Nursing Assistant to join our caregiving team. You will support nurses, assist residents with daily living activities, and help maintain care plans with empathy and attention to detail.

The role involves lifts and transfers, meal assistance, documentation, weight monitoring, skin care, and collaboration with a multidisciplinary team. We offer PTO, health benefits, a 401(k), and tuition reimbursement to eligible staff.

Qualifications

• Certified Nursing Assistant with good standing or graduate of state-approved CNA program.
• High School Diploma or equivalent preferred.
• Current CPR/BLS certification preferred.
• At least one year of CNA experience in long-term care preferred.
• Effective written and verbal communication in an interdisciplinary team.

Responsibilities

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living according to care plans.
• Assists with lifting, turning, moving, positioning, and transporting residents.
• Coordinates dining room services at mealtimes, including setup, tray delivery, feeding, and documenting meal intake.
• Delivers nutritional supplements and ensures intake is documented.
• Assists with weighing residents and reports weight changes.
• Monitors skin condition and reports any pressure areas or skin issues.
• Assists with toileting program activities.
• Completes flow sheets and documents resident progress daily.
• Keeps supervisor informed of factors affecting work performance.
• Reports changes in resident condition to nurse and supervisor.

A Certified Nursing Assistant is responsible for supporting medical staff in administering care to residents with various injuries, illnesses or disabilities. The position requires patience, compassion, and a desire to care for the elderly in a gentle and empathetic manner.

Responsibilities and Duties

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Qualifications and Skills

• Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a certified nursing assistant in long-term care preferred.
• Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
