Responsibilities And Duties

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists residents with activities of daily living according to care plans and policies.
• Assists with lifting, turning, moving, and transporting residents.
• Coordinates dining services, including set-up, clean-up, meal tray delivery, and feeding assistance.
• Delivers nutritional supplements and documents intake.
• Assists with weighing residents and reports changes.
• Tracks skin condition and reports issues.
• Supports toileting activities.
• Completes flow sheets and documents resident progress.
• Informs nurse and supervisor of work-related factors.
• Reports changes in residents’ conditions.

Qualifications And Skills

• Certified Nursing Aide in good standing or graduate of approved training program with passing exam (within 120 days of employment) REQUIRED
• High School Graduate/GED preferred
• Current CPR/BLS certification preferred
• At least one year of long-term care experience preferred
• Effective communication skills in written and oral form
