Contract: 13 weeks

Facility: Mohawk Valley - Rehabilitation and Nursing Center (RNC)

Bill Rate: $60

Local Rate: Reduce $7 from the bill rate (applicable for locals within 75 miles)

Job Description:

Assist in providing quality care to residents in a long-term care (LTC) setting, including patients with diagnoses such as MS, Orthopedic issues, Dementia, and Memory Care. Responsible for supporting nursing staff and ensuring resident comfort and safety.

Submission Requirements:

• Up-to-date resume with 1+ years of experience in an LTC facility and documented travel experience
• Two recent Charge/Manager References
• Skills Checklist matching the specialty
• Drivers license with address matching demographics
• Current BLS certification
• NURSYS report verifying active NY LPN license and all active licenses
• RTO included at time of submission
• EMR experience with PointClick Care

Additional Details:

• Shift: 10:30 PM - 6:30 AM (Block scheduling not guaranteed)
• EOW (Every Other Weekend) required
• Floating within the same building
