Job summary

Three Oaks Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team providing in-home patient care. You will deliver personal healthcare services in the patient’s residence, maintain a safe, clean environment, and collaborate with families and the supervisor.

Work under the direction of a registered nurse in a supportive hospice network. Ideal candidates have CNA licensure, state-approved nurse aide training, and at least one year of hospice or long-term care

Qualifications

• High School Diploma preferred.
• State-approved nurse aide training/competency program.
• Must have CNA license.
• Must have a valid driver's license.
• At least one year of hospice or long-term care experience preferred.
• Reliable transportation.

Responsibilities

• Provide personal healthcare and related services in the patient’s home.

• Work under the direction of a registered nurse and coordinate with family.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
