Responsibilities

• Report any unusual circumstances in the patient's condition or home environment to the primary RN/Patient Care Manager.
• Follow the current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs), including mobility transfer, walking, grooming, bathing, dressing or undressing, eating, and toileting.
• Perform incidental household services essential to the patient's healthcare at home, as assigned, to help prevent or delay institutionalization.
• Complete a clinical note for each visit within the required timeframes, ensuring it is incorporated into the patient's record.

Education and Experience

License Requirements

• Successful completion of a competency evaluation.
• Current CPR certification required.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
• Minimum of one year of full-time experience in direct client care.
• Mature and able to handle the demands of a home health aide position. Capable of reading, writing, and following directions promptly and accurately.
