As a part of the Telecare Family, La Paz is a 148 bed Gero-Psychiatric center which serves adults 55 and older.

Shifts Available:

1. AM 7:00 am - 3:30 pm / PM 3:30 pm - 11:00 pm / NOC 11:00 pm - 7:30 am
2. On call; Shift hours and days vary as needed and/or based on candidate availability

Expected starting wage range is $21.49 - $26.25 hourly.

We pay differentials! Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift).

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

POSITION SUMMARY

The Certified Nursing Assistant provides care in Skilled Nursing Facilities that responds to the physical and psychological needs of members served and is consistent with recovery-centered care principles.

QUALIFICATIONS

Required:

• High School Diploma or GED
• Current Nursing Assistant Certification
• Ability to communicate in English both verbal and written
• Must be at least 18 years of age
• Must be CPR, CPI, and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
• All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, motor vehicle clearance, and proof of auto insurance are required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply.

Preferred:

• Previous experience with the population to be served and/or work in a similar program setting
• Bi-lingual for threshold languages

ESSENTIAL FUNCTIONS

• Demonstrate the Telecare mission, purpose, values, and beliefs in everyday language and contact with stakeholders
• Assist in the welcome, admission, and discharge processes
• Support and coach members in activities of daily living
• Ensure safety of members through monitoring and observation; complete related documentation
• Monitor and assist members at mealtime
• Attend and participate in community meetings and groups
• Participate in and facilitate rehabilitation therapy groups and activities, as needed
• Implement treatment of care plans
• Create a recovery environment through interactions with staff and members
• Collect stool, urine, and other specimens as requested
• Obtain vital signs and weights, as assigned
• Practice standard precautions at all times
• Participate actively in multidisciplinary team and treatment planning meetings
• Maintain confidentiality and integrity of client information and records
• Demonstrate knowledge of CPI principles and techniques
• Inform charge nurse of observed behavior and conditions
• Observe, record, and report client behavior; recognize changes and modify care accordingly
• Review admission documentation and assist in assessment data collection
• Assist with restraint of members in case of assaultive behavior and pass relevant training

Additional duties may be added, deleted, or changed at management's discretion.

PHYSICAL DEMANDS

The physical demands include sitting, bending, squatting, kneeling, crawling, climbing, lifting (up to 25 pounds), walking, standing, twisting, reaching, and grasping. Exposure to uneven ground and hazardous materials may occur. Visual requirements include working with computers and books.
