Job summary

Santè is seeking a Certified Nursing Assistant to perform personal care for patients in their residence under RN supervision. You will assist with admissions, transfers and discharges, ensuring care plans are followed and personal needs are met.

Qualifications

• CNA license certification required before hiring.
• Current CPR and fingerprint clearance card required.

• Recent Long-Term Care / Skilled Nursing Facility or Transitional Rehab experience is preferred.
• LTC/SNF/Rehab experience preferred.

Responsibilities

• Assist guests/patients during admissions, transfers and discharges as per care plan.
• Review care plans daily to determine changes in the daily care routine.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience)
• LTC/SNF/Rehab preferred.
