Job summary

Santè is seeking a Certified Nursing Assistant to join our patient-centered care team in Chandler, AZ. As a CNA, you will provide essential personal care to residents, assist with admissions and transfers, and support the care plan under RN supervision.

Qualifications

• CNA license (required) and CPR certification.
• Fingerprint clearance required.
• Minimum six months CNA/LNA experience preferred.
• Recent LTC/SNF or Transitional Rehab experience preferred.
• CPR and fingerprint clearance must be current.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patient’s personal care needs are met per the care plan.
• Review care plans daily to determine changes in the guest/patient’s daily routine and care plan.

Certified Nursing Assistant is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN).

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guest/patient's daily care routine have been made on the care plan.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience)
• LTC/SNF/Rehab preferred.
