You will assist residents with ADLs, meals, grooming, mobility and rehabilitation activities, delivering supportive, person-centered care daily. Under supervision, you will report changes in residents' condition, maintain infection control, document care, and participate in care conferences.

Qualifications

  • High School diploma or GED preferred.
  • Successful completion of a state-approved Nursing Assistant program; must obtain and maintain mandatory state and federal requirements.
  • Ability to operate Sit to Stand, Hoyer Lift, gait belts, tub chairs.
  • Ability to read, write, understand and speak English; use computer for charting purposes.

Responsibilities

  • Participates in nursing report at duty start.
  • Assists residents with bathing, grooming, hygiene and daily activities.
  • Assists with meals, hydration and feeding as needed.
  • Reports changes in residents' condition to supervisor promptly.
  • Maintains cleanliness of resident units and storage areas.
  • Documents entries on flow sheets, I&O, and other care records at point of care.
  • Supports residents for appointments and activities; follows individual care plans.
  • Ensures safety and infection control guidelines are followed.

Community Name:Trinity Community at BeavercreekThe Certified Nursing Assistant (CNA)/ Competency Evaluated Nursing Assistant (CENA) is responsible for caring for residents both physically and emotionally to enhance quality of life, including ADL and rehabilitation activities.Essential Function Statement(s)Participates in and receives nursing report upon reporting for dutyAssist residents at mealtimesAssists with resident care including bathing, grooming, hygiene and placement of adaptive equipmentExplains procedures to residents before administering, drapes residents for examination and treatment.Keeps residents dry (changes clothing, linen, etc, when wet or soiled)Assists residents with bowel and bladder functionsEmpties urinary drainage bags and records amount in record; Reports any irregularities to nurseAssists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., keeping with specific resident safety needs; Assists residents in ambulation, with or without self-help devices, as instructedPerforms restorative and rehabilitative procedures, as instructedEnsures call lights are within reach of residents and answers call lights promptlyEnsures residents’ personal care needs are being met in accordance with residents’ wishesReports all changes in residents' condition to supervisor as soon as practicalChanges bed linens and keeps linens tight to avoid wrinkles from forming under residents; Handles linens according to infection control guidelinesMaintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, linen closetsAssists residents in preparation for and transporting to appointments, activities and social programsUses creative problem solving to meet the needs of the resident, within scope of responsibilityProvides post-mortem care as instructedPerforms required special nursing care duties, including identifying and reporting abnormal skin conditionsEnsures residents are positioned and prepared appropriately for meals; serves food and assists with feeding as necessary; ensures each resident is properly hydrated; assists in the delivery and documentation of all snacks and supplementsAccurately records all entries on flow sheets, I and O's, and other documents at point of care as requiredParticipates in care conference as indicated and follows individual resident care plansPerforms all admission, discharge and transfer duties as requiredMaintains confidentiality of necessary informationUtilizes supplies and equipment properly and without waste, including restocking suppliesReports observations concerning structural, equipment and furniture defects and malfunctioning to appropriate personnelHandles unusual occurrences calmly and logically to maintain continuity of dutiesMaintains appropriate, effective communication with residents, and with residents' family or other significant relationships, and ensures departmental staff does the sameFollows all appropriate safety and security guidelines, care procedures and protocol for residents in the Dementia Care UnitParticipates in relevant educational and training activities as appropriateParticipates in special activities as necessary (e.g., safety committee;
