The position supports residents with daily living activities, mobility, vital signs, and documentation, while collaborating with nurses and physicians to deliver cohesive care. This is a Mon-Fri 0630-1430 role in a facility-focused environment.

Qualifications

• Experience in a skilled nursing facility or similar healthcare setting preferred.
• Ability to lift and move residents and perform physical tasks associated with patient care.
• Current CNA certification in Nebraska required.

Responsibilities

• Assist residents with daily living activities including bathing, dressing, grooming, and personal hygiene.
• Help residents with mobility including transferring, positioning, and ambulating safely.
• Take and record vital signs such as blood pressure, temperature, pulse, and respiration rates.
• Accurately document all care provided and any changes in residents' condition in electronic health records.
• Report changes or concerns to nursing staff promptly.
• Assist with feeding, preparing meals, and providing companionship.
• Follow infection control protocols and maintain a clean environment.
• Collaborate with nurses and physicians to ensure cohesive care.
