Job summary

Bronte Health & Rehab Center is seeking a Certified Nursing Assistant to join our skilled nursing team. You will support nurses and staff in delivering high-quality resident care and assist with daily living activities according to individualized care plans.

Responsibilities include entreging meals, documenting intake, weighing residents, monitoring skin condition, and reporting changes to the nurse and supervisor. A state CNA license, HS diploma, and CPR/BLS certification are required.

Qualifications

• Certified Nursing Assistant in good standing with the state.
• High school diploma or equivalent.
• Current CPR/BLS certification.
• Ability to work as part of a team and communicate effectively with residents, families, and staff.
• Ability to lift, move, and position residents and assist with transfers and other physical activities.

Responsibilities

• Provide certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures, and at the direction of supervisor(s).
• Assist nurse(s) and other staff as needed, and perform duties as assigned.
• Assist residents with activities of daily living per care plans and policies.
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Coordinate dining room services at assigned mealtimes, including setup, delivery, and documentation of meal intake.
• Deliver nutritional supplements to residents and document intake.
• Weigh residents and report changes to nurse/supervisor.
• Track skin condition and report pressure areas or tears.
• Assist with toileting program activities.
• Complete flow sheets daily and report progress to supervisor.
• Report factors affecting performance to nurse/supervisor.

Responsibilities:

• Provide certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures, and at the direction of supervisor(s)
• Assist nurse(s) and other staff as needed, and perform duties as assigned
• Assist residents with or perform activities of daily living for resident in accordance with care plans and established policies and procedures
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinate dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake
• Deliver nutritional supplements to residents at assigned times and provide assistance as necessary to ensure intake, documenting intake accordingly
• Assist with weighing residents according to facility policy, and record weight in designated locations, reporting weight changes to nurse and supervisor
• Assist with tracking the condition of the resident's skin, reporting any presence of pressure areas, skin breakdown, or skin tears to nurse and supervisor
• Assist nursing staff in carrying out toileting program activities
• Complete flow sheets daily to indicate that the specified task was done, documenting the progress of each resident as directed by supervisor
• Keep nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit)
• Report changes in a resident's condition to the resident's nurse and supervisor

Requirements:

• Certified Nursing Assistant in good standing with the state
• High school diploma or equivalent
• Current CPR/BLS certification
• Ability to work effectively as part of a team, and to communicate effectively with residents, families, and staff
• Ability to lift, move, and position residents, and to assist with transfers and other physical activities
