Qualifications

• Attend work as scheduled and follow company’s call-off procedures.
• Document incidents and/or unusual problems according to facility procedures.
• Observe infection control procedures.

Responsibilities

• Attend work as scheduled and follow company’s call-off procedures.
• Identify resident problems and report to a charge nurse or licensed nurse.
• Complete assignments timely, completely and accurately.
• Conduct resident rounds as assigned.
• Document in the nursing assistant notes the care provided and resident response.
• Report changes in residents’ conditions to Charge Nurse or Nurse Supervisor.
• Follow facility procedures for admission, discharge and transfer of residents.
• Assure residents have call lights and respond promptly.
• Assist residents with activities of daily living and meals as per care plans.
• Check name bands daily and report any changes.
