Grace Skilled Nursing & Therapy in Jenks, Oklahoma is seeking a compassionate Certified Nursing Assistant (CNA) to join our specialized respiratory and tracheostomy unit. You will provide direct resident care in a high-acuity environment, support ADLs, and collaborate with an interdisciplinary team to ensure comfort and quality of life.

Qualified candidates hold an Oklahoma CNA certification in good standing and a current BLS certificate.

Qualifications

Responsibilities

Certified Nursing Assistant (CNA) | Specialized Respiratory & Tracheostomy Unit | Jenks, Oklahoma

Are you an experienced Certified Nursing Assistant (CNA) with a background in LTAC, step-down ICU, tracheostomy care, or other high-acuity settings? We are seeking a compassionate and clinically skilled CNA to join our specialized respiratory and tracheostomy unit.

This unique opportunity offers the chance to care for residents with complex respiratory and medical needs in a dynamic, clinically complex environment. Working alongside an experienced interdisciplinary team, you will utilize and further develop your clinical skills while supporting residents’ comfort, dignity, well-being, and quality of life.

Resident Care & Activities of Daily Living (ADLs)

• Assist residents with activities of daily living, including bathing, bed baths, CHG bathing, grooming, oral care, dressing, toileting, feeding, and incontinence care.
• Deliver meals, provide hydration, and follow prescribed aspiration precautions and care instructions while assisting with feeding.
• Turn and reposition residents according to established care plans and schedules to promote comfort and support skin integrity.
• Assist residents with mobility, transfers, ambulation, and range-of-motion activities.
• Support physical, occupational, speech, and respiratory therapy teams during treatment sessions, as appropriate.
• Provide comfort measures and other non-sterile supportive care interventions consistent with training and facility policy.

Resident Monitoring & Documentation

• Obtain and accurately document vital signs, including blood pressure, pulse, respirations, oxygen saturation, and temperature.
• Measure and record intake and output, including enteral feeding intake and drainage measurements, as directed.
• Obtain and document daily weights and other assigned observations.
• Recognize and promptly report changes in resident condition, behaviors, or abnormal findings to the licensed nursing team.
• Maintain accurate, timely, and complete documentation in accordance with facility policies and regulatory requirements.

Respiratory Support

• Assist licensed nursing staff with tracheostomy-related support activities, including routine observation, comfort measures, and other tasks permitted by state regulations, facility policy, and demonstrated competency.
• Observe oxygen delivery equipment, including tracheostomy collars, nasal cannulas, and oxygen tubing, and promptly report concerns or changes to licensed nursing staff.
• Observe and immediately report signs of respiratory distress, including changes in breathing patterns, oxygen saturation, comfort level, or secretions.

Skin Integrity & Wound Support

• Observe and report skin concerns identified during routine care activities.
• Follow established repositioning schedules and preventive measures to support skin integrity.
• Assist licensed nursing staff with preparation for wound care procedures and non-sterile dressing changes, as directed.
• Communicate resident concerns and changes in condition promptly to the nursing team.
• Collaborate with nurses, therapists, and other interdisciplinary team members to support high-quality resident care.
• Help maintain a clean, safe, and supportive environment for residents, families, visitors, and fellow team members.

Required Qualifications

• Current Oklahoma Certified Nursing Assistant (CNA) certification in good standing.
• Current Basic Life Support (BLS) certification.
• Strong communication, observation, and resident care skills.
• Ability to successfully complete all pre-employment screenings in accordance with applicable laws and regulations.
• Ability to lift, bend, stoop, stand, walk, and assist with resident transfers, with or without reasonable accommodation.

Preferred Qualifications

• One (1) or more years of experience in LTAC, step-down ICU, respiratory care, skilled nursing, post-acute care, or another high-acuity healthcare setting.
• Experience caring for residents with tracheostomies, oxygen therapy, enteral feeding, or other complex medical needs.
• Experience working in skilled nursing, rehabilitation, hospital, LTAC, or other post-acute healthcare environments.
• Demonstrated ability to thrive in a dynamic, interdisciplinary care setting.
