Comprehensive ADL & Personal Care Assistance:
Provide hands-on assistance with essential Activities of Daily Living (ADLs), including bed baths, showers, whirlpools, dressing, personal grooming, oral care, and incontinence management.
Serve nutritional meals, assist residents with dining, provide hydration reminders, and carefully monitor and record daily caloric and fluid intake.
Ensure residents are treated with utmost dignity, respect, and emotional warmth during all personal care interactions.
Clinical Partnership & Resident Observation:
Accurately measure, record, and track vital signs (temperature, pulse, respiration, blood pressure, and weights) according to resident care plans.
Serve as a vital link on the nursing team by detecting subtle physical, emotional, or cognitive changes such as skin redness, acute pain, changes in mobility, or appetite loss and reporting findings immediately to the Charge Nurse.
Mobility, Safety & Infection Control:
Safely assist residents with transfers, ambulation, turning, and positioning using proper body mechanics and adaptive equipment (Gait belts, Hoyer lifts, sit-to-stand lifts).
Support restorative nursing programs and physical therapy maintenance routines to promote resident strength and independence.
Adhere strictly to hand hygiene standards, PPE guidelines, standard precautions, and facility infection control protocols.
Digital Documentation & Handoff Communication:
Maintain timely, thorough, and accurate digital charting in the Electronic Health Record (EHR) kiosk system, logging meals, fluid intake, mobility levels, and care delivered prior to shift completion.
Participate actively in shift huddles and communicate key resident details during shift handoffs to ensure seamless care transitions.
Certification: Must possess an active Wisconsin CNA Certified Nursing Assistant registration in good standing with the state registry.
Education: High School Diploma or GED equivalent required.
Tech Skills: Basic computer skills required for electronic care charting and internal facility communication.
