The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed.**Hours / Schedule:** Full Time; (40) hours / week. Works in patients' home in various conditions. Ability to work flexible schedule, ability to travel locally and ability to participate in holiday and weekend rotation as appropriate.**Minimum Qualifications:*** High school graduate or equivalent.* Meet the training and certification requirements of the State.* Possess and maintain current CPR certification.* Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.* Able to show empathy for the needs of the ill, injured, frail and the impaired.* Prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.**Key Responsibilities:*** Perform personal care needs after given a patient's assignment and attending to his/her requests promptly.* Meet the safety needs of patients and use equipment safely and properly (foot stools, side rails, O2 etc.)* Perform personal care tasks including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.* Assist in dressing and undressing patients as assigned.* Assist in feeding the patient as assigned.* Assess and report on patients’ condition and significant changes to the case manager.* Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.* The hospice aide will not function in any manner, viewed as the practice of nursing according to the Nurse Practice Act.
