Job summary

Longwood Community Living Center is seeking a Certified Nursing Assistant (CNA) to join our team on a PRN schedule. The CNA will perform direct care, monitor residents, and assist with activities of daily living while maintaining a safe, clean, and compassionate environment.

The role requires a valid CNA license in good standing, ability to coordinate with the care team for admissions, transfers, and discharges, and commitment to infection prevention and high-quality resident care.

Qualifications

• Certified Nursing Assistant (CNA) license in good standing required
• Ability to perform direct care and monitor residents' needs
• Competent in infection prevention and control practices

Responsibilities

• Conduct regular direct care rounds to assist with activities of daily living
• Obtain vital signs, weights, and heights as instructed
• Coordinate admissions, transfers, and discharges in the Transitional Care Unit
• Notify Charge Nurse of any resident/patient changes in condition
• Maintain safe and clean work area and ensure infection control practices
• Complete required documentation and duties as assigned
• Build positive relationships with residents and families

PRN schedule

Certified Nursing Assistant ( CNA ) duties include but are not limited to:

• Conduct regular direct care rounds on assigned unit to monitor activity and ensure residents needs are met including assisting with activities of daily living
• Maintain positive relationships with the residents/ Patients
• Notify Charge Nurse of any resident/ patient change in condition
• Obtain vital signs, weights, and heights as instructed by charge nurse
• Complete all assigned duties and documentation as required
• Coordinate and prepare for new admissions, transfers, discharge of residents in the Transitional Care Unit
• Maintain safe and clean work area
• Ensure infection prevention and control practices
• Other duties as assigned
