Job summary

The Care Team Hospice in Decatur, IL is seeking a compassionate Certified Nursing Assistant (CNA) to join our patient-focused team. You will provide hands-on personal care and comfort to hospice patients while supporting families through life's important moments.

This role offers a daytime schedule with no nights or weekends, mileage reimbursement, and comprehensive benefits. You will collaborate with an experienced interdisciplinary team to promote dignity, comfort, and quality of life at home

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
• Valid driver's license, reliable transportation, and current automobile insurance.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
