Job summary

Elara Caring seeks a dedicated Certified Nurse Aide to join our home health team, delivering compassionate, patient-centered care in clients' homes across Michigan. You will work under a Registered Nurse, assisting with daily activities, monitoring vital signs, and supporting safe mobility while honoring individualized care plans.

Responsibilities include accurate documentation, medication reminders, safety protocols, and effective communication with the care team.

Qualifications

• Current State Home Health Aide or Certified Nursing Aide certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.

• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.
